These services can include:

  • Child Care Services
  • Personal Supports
  • Household Services
  • Home Modification
  • Assistive Technology
  • Recreational Equipment
  • Specialized Medical Equipment and Supplies
  • Professional Services
  • Behavioral Services
  • Community Connection Service

Further examples of potential services are available through the CES Case Manager or Program Manager at Community Connections.

Eligibility for CES

(ref. DDD rule 16.600, HCPF rules 8.503.30)

In addition to being determined eligible as having a developmental disability, a child applying for CES services must meet additional criteria to enroll in the CES program. Additional requirements include that the child be under the age eighteen (18) and living with his/her biological or adoptive family, meet SSI criteria for a disability, meet financial eligibility for a Medicaid Waiver program and meet the targeting criteria for CES. The targeting criteria require that the child demonstrates a behavior or has a medical condition that requires direct human intervention, more intense than a verbal reminder, re-direction or brief observation of medical status, at least once every two hours during the day and on a weekly average of once every three hours during the night.

The Community Connections CES Case Manager can assist families through the application process.
